Painful diabetes complication now being treated with established therapy option

(BPT) – Living with diabetes had always been manageable for Kent Apple by eating healthy and remaining active. However, he knew he needed to see his doctor when he started experiencing pain in his legs and feet.

“As a retired construction worker, I noticed the pain the last several years that I was working and thought it was from walking on concrete all day,” said Apple. “The pain felt like an electric shock in my feet and legs.”

He was suffering from painful diabetic peripheral neuropathy (DPN) — a chronic pain condition that affects approximately 30% of diabetes patients.1 High blood sugar levels can damage nerves over time, leading to pain. For some, the pain can become excruciating and negatively impact quality of life.

Those who suffer from DPN often experience chronic numbness, burning or stabbing pain in their legs and feet, making it difficult to walk, stand or even sleep.

There is no cure for DPN, and treatment is directed at slowing the worsening of symptoms, relieving pain and managing complications.

“In my practice, I always ask my patients if they have diabetes or symptoms of DPN because diabetes is a very common medical problem,” said Dr. G. Silky Patel, an interventional spine, sports and pain management physician in Houston, Texas. “When a patient is referred to me for DPN, we start with treatments like gabapentin, compression socks, pain creams and physical therapy, but those don’t always work.”

While mild and moderate cases of DPN can be treated with over-the-counter or prescription medications, some data suggests limited effectiveness. One study showed approximately 77% of patients prescribed an anticonvulsant (pregabalin) for DPN will discontinue the treatment within 1 year because of intolerable side effects or lack of efficacy.2,3 More severe DPN cases may be treated with opioids, which have potentially severe side effects, including addiction and abuse.1

To help treat Apple’s painful DPN, Dr. Patel first recommended gabapentin. While it helped, the relief didn’t last, and Dr. Patel had to increase the dose. Apple found himself back in her office just a few months later, complaining of the side effects of gabapentin. Apple felt he was at a loss until Dr. Patel gave him information on spinal cord stimulation.

Spinal cord stimulation (SCS) is a proven, non-opioid, FDA-approved way to manage chronic pain that is caused by DPN. The technology has been around for about 40 years, and works by disrupting the pain signals traveling between the spinal cord and the brain. Stimulation is delivered via a Medtronic Intellis or Vanta neurostimulator, implanted under the skin. These Medtronic spinal cord stimulators were recently approved by the U.S. Food and Drug Administration for the treatment of pain caused by DPN.

“At first, I was a little hesitant. Being 73, the thought of having something implanted in my body was a little scary,” said Apple. “But now I know what a difference SCS has made for me, and it was no mistake at all.”

Studies show people with painful DPN are 17 times more likely to experience significant pain relief if treated with SCS compared to conventional treatment.4,5,6 Additionally, 86% of patients experienced treatment success after receiving SCS therapy for one year and benefited from meaningful pain relief out to 5 years.7

“I’ve seen how DPN patients have benefitted greatly from SCS implants,” said Dr. Patel. “It’s important that my DPN patients, who have been told that there’s no hope for their pain symptoms, know that there are treatments out there that are proven to effectively provide therapeutic relief.”

SCS can offer proven long-term therapy for managing chronic pain, with the majority of patients experiencing meaningful pain relief through five years of treatment using the device.7

This treatment isn’t made for everyone, and these decisions should be made in consultation with a doctor. Well-child visits help you prioritize your child’s physical and mental health

(BPT) – Annual well-child visits are doctor appointments for preventive health services which are essential for ensuring a child’s growth and tracking developmental milestones. The well-child visit is also the time for routine immunizations to prevent diseases like measles, polio, hepatitis B, chickenpox, whooping cough and other serious diseases.

“Like vaccines, which prevent physical health conditions, speaking with your child’s primary care physician regularly about mental health concerns is also an essential part of overall preventive care,” said Rhonda L. Randall, D.O. and chief medical officer at UnitedHealthcare. “Your annual well-child visit is also an opportunity to have a conversation with your child’s physician. It’s best to have these conversations when problems or warning signs first appear, so your physician can take the appropriate steps to best treat them.”

If you’re not sure what questions to ask your child’s primary care physician during an annual well-child visit, consider the following:

1) Ask what vaccines are appropriate for your child’s age — and how to make up any that have been missed. You can refer to the list of child and adolescent vaccines recommended by the Centers for Disease Control (CDC) at CDC.gov/vaccines. In addition to other childhood vaccines, both flu and COVID-19 vaccines are recommended by the CDC for everyone 6 months of age and older. If you are concerned about childhood vaccines, ask the pediatrician about common side effects, which are typically very mild, such as pain or swelling at the injection site, and can include low-grade fever or rash.

2) Discuss changes in your child’s behavior. Some common warning signs that your child’s mental well-being isn’t where it needs to be include persistent sadness, withdrawing from or avoiding social interactions, displaying outbursts of extreme irritability, drastic changes in mood, behavior or personality, changes in eating habits, difficulty sleeping, frequent headaches or stomachaches, difficulty concentrating, changes in academic performance or avoiding or missing school.

3) Ask for guidance on how best to support your child. Whether you have concerns about your child’s nutrition, exercise, sleeping patterns or behavioral changes, your child’s primary care physician is a great place to start. With so many young children experiencing mental and emotional health challenges, it’s important to create opportunities for them to share how they are really doing. Remember that these can be sensitive topics for your child to discuss. Empathy and patience go a long way to help children and adolescents feel listened to and comfortable.

4) Don’t forget to bring your sports physical forms. It’s great if your child participates in school sports. The wellness visit is an opportune time to make sure your physician is aware that your child is a student-athlete and address any concerns like nutrition, prior injuries and family history.

5) Ask for recommendations for other health care professionals, if needed. For example, if your child hasn’t seen the dentist in a while, if their vision screening indicated that they need to see an eye doctor, or their mental health screening has raised concerns, ask which health care professionals in your plan’s network they would recommend.

According to the Global Carer Well-Being Index, a survey of more than 9,000 global caregivers released by EMD Serono in 2021, 89% of caregivers said they put the needs of those they are caring for over their own; 55% said they are not getting enough sleep; and 45% said they are exercising less than they were before the pandemic.

The Carer Well-Being Index underscored the mental health challenges many caregivers face. Caregivers struggled with their mental health long before COVID-19, but the pandemic just exacerbated those issues. Now, caregivers need support more than ever.

“Self-care for caregivers is not a ‘nice-to-have,’ it is an essential,” said Hazel Moran, vice president of Global Patient Insights and Advocacy of EMD Serono. “We can help caregivers prioritize their mental health by making support more readily available and encouraging them to practice a routine of self-care early on in their caregiving journey.”

Research shows that when the mental health of the caregiver is compromised, the outcomes of those for whom they are caring are affected. It can be hard to know how to support caregivers, but here’s a good way to get started.

In speaking with caregivers like Wanda, who voiced their challenges, here are five caregiver-approved tips for how to practice self-care:

1) Disconnect in ways that feel right for you

Find what recharges you and brings you joy outside of caregiving and take time to do it. As an unpaid caregiver, you don’t get to clock out or take vacation days. And when you naturally put the needs of your loved one above your own, taking a break can feel selfish. It’s not.

Disconnecting is necessary to keep you healthy and at your best. It can be as simple as check-ins with yourself to make sure you’re eating and sleeping regularly. It can also be as big as asking someone to step in while you take a break or go on a vacation.

2) Acknowledge your limitations and listen to your own needs

Recognizing your own limitations is the first step toward avoiding burnout, because taking care of someone 24/7 and not taking care of yourself isn’t sustainable. Note the emotions you are holding back and accept that you’re not a machine and that you too deserve some care.

3) Communicate more with those around you

Many caregivers often feel that it’s difficult for others to understand what they’re going through, as many healthcare professionals, friends and family members don’t know what it’s really like. Talking things through with these people not only gives people a better picture of what’s going on, it can also help carve a path toward real solutions.

4) Seek mental health support

Sometimes it’s hard to manage the feelings of guilt, helplessness and sadness that come with caring for a loved one.
