From struggle to success: One woman’s weight loss journey

(BPT) – If you’ve had difficulty managing your weight throughout your life, you’re certainly not alone. Like project manager Janet, many people search for positive solutions to reach and maintain a healthier weight, but often find themselves in a spiral of self-defeating thoughts that can be hard to overcome.

I’ve struggled with managing my weight my entire life,” recalled Janet. “My eating behavior led to decades of weight gain, accompanied by physical and emotional pain.”

Seeking to improve her situation, Janet hoped that her healthcare providers could be a helpful resource, but found herself disappointed by their responses. “For years, I sought help from my doctors, only to be regularly shut down with my concerns. I felt embarrassed that my struggles were being dismissed by someone who was supposed to be helping me, yet couldn’t even look me in the eye when discussing weight,” Janet said. “They never took the time to help me; it was always ‘just eat right and exercise.’”

Because she was unable to find specific direction and help from her doctors, Janet often felt worse about herself. “Stress was a main trigger for my eating as there was so much to juggle between working full-time, raising children and family obligations. When I felt disappointed because something wasn’t going right for me, I would spiral into a negative mindset. I would start to think that nobody cared if I took care of myself or not, or that I deserved snacks because of a situation, and that eating would make me feel better,” Janet explained. “These thought patterns only prompted me to eat more. I could easily power through bags of cookies and chips with no stop button in sight.”

Fortunately, she found a healthcare provider who took her concerns seriously and helped her come up with concrete solutions. I felt like I was being given up on until I finally found a nurse practitioner who listened to me and who was truly on my side,” said Janet. “She was the one who finally told me there is something else I can do alongside diet and exercise. That’s when I found out about the medication CONTRAVE® (naltrexone HCI/bupropion HCl).”

What is CONTRAVE?

The weight-loss journey

“My journey wasn’t easy. Tell your healthcare provider about all of your medical conditions including if you have: depression or other mental illnesses; attempted suicide; seizures; head injury; tumor or infection of brain or spine; low blood sugar or low sodium; liver or kidney problems; high blood pressure; heart attack, heart problems, or stroke; eating disorder; drinking a lot of alcohol; prescription medicine or street drug abuse; are 65 or older; diabetes; pregnant or planning to become pregnant; or breastfeeding.

Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.

CONTRAVE may cause serious side effects, including:

  • Seizures. There is a risk of having a seizure when you take CONTRAVE. The risk of seizure is higher in people who take higher doses of CONTRAVE, have certain medical conditions, or take CONTRAVE with certain other medicines. Do not take any other medicines while you are taking CONTRAVE unless your healthcare provider has said it is okay to take them. If you have a seizure while taking CONTRAVE, stop taking CONTRAVE and call your healthcare provider right away.

  • Risk of opioid overdose. Do not take large amounts of opioids, including opioid-containing medicines, such as heroin or prescription pain pills, to try to overcome the opioid-blocking effects of naltrexone. This can lead to serious injury, coma, or death.

    Get emergency medical help right away if you take opioids and you:

    • have trouble breathing
    • become very drowsy with slowed breathing
    • have slow, shallow breathing
    • feel faint, very dizzy, confused, or have unusual symptoms
  • Sudden opioid withdrawal. People who take CONTRAVE must not use any type of opioid including street drugs, prescription pain medicines, cough, cold, or diarrhea medicines that contain opioids, or opioid dependence treatments, for at least 7 to 10 days before starting CONTRAVE. Using opioids in the 7 to 10 days before you start taking CONTRAVE may cause you to suddenly have symptoms of opioid withdrawal when you take it. Sudden opioid withdrawal can be severe, and you may need to go to the hospital. Tell your healthcare provider you are taking CONTRAVE before a medical procedure or surgery.
  • Severe allergic reactions. Stop taking CONTRAVE and call your healthcare provider or go to the nearest hospital emergency room right away if you have any of the following signs and symptoms of an allergic reaction: rash, itching, hives, fever, swollen lymph glands, painful sores in your mouth or around your eyes, swelling of your lips or tongue, chest pain, or trouble breathing.
  • Increases in blood pressure or heart rate. Some people may get high blood pressure or have a higher heart rate when taking CONTRAVE. Your healthcare provider should check your blood pressure and heart rate before you start taking, and while you take CONTRAVE.
  • Liver damage or hepatitis. Stop taking CONTRAVE and tell your healthcare provider if you have any of the following symptoms of liver problems: stomach area pain lasting more than a few days, dark urine, yellowing of the whites of your eyes, or tiredness. Your healthcare provider may need to stop treating you with CONTRAVE if you get signs or symptoms of a serious liver problem.
  • Manic episodes. Bupropion can cause some people who were manic or depressed in the past to become manic or depressed again.
  • Visual problems (angle-closure glaucoma). Signs and symptoms may include: eye pain, changes in vision, swelling or redness in or around the eye. Talk with your healthcare provider to find out if you are at risk for angle-closure glaucoma and to get treatment to prevent it if you are at risk.
  • Increased risk of low blood sugar in people with type 2 diabetes mellitus who also take medicines to treat their diabetes (such as insulin or sulfonylureas). You should check your blood sugar before you start taking CONTRAVE and while you take CONTRAVE.

The most common side effects of CONTRAVE include nausea, constipation, headache, vomiting, dizziness, trouble sleeping, dry mouth, and diarrhea. These are not all of the possible side effects of CONTRAVE.

A rare strength: What one advocate wants you to know about the rare disease community

(BPT) – By definition, the term “rare” implies something uncommon. When applied to diseases, it means a condition that impacts a small number of patients, usually fewer than 1 in 2,000. However, to have a rare disease, and the challenges it brings, is more common than many people may realize.

Diane Ramsey-Paige — a rare disease patient, caregiver, advocate and nurse for more than 40 years — knows this all too well.

Ramsey-Paige lives with two different rare blood disorders, and her youngest child has an ultra-rare immune regulation syndrome. Even with her training as a nurse and experience as a rare disease patient herself, navigating her son’s rare disease journey was a challenge.

“Despite having a physician and advanced practice nurse for parents and being evaluated at a world-class medical center, it took quite some time to arrive at a diagnosis and a treatment plan for our son,” said Ramsey-Paige. “Due to a variety of factors, this experience is unfortunately quite common among those with rare disorders.”

A calling to help

A lack of education in the healthcare community and awareness about these rare conditions can delay time to diagnosis, just as it did for Ramsey-Paige’s son. Some patients can wait years or longer for an appropriate diagnosis, a process that can take a toll. Other challenges include lack of available treatment once diagnosed, since 95% of rare diseases don’t have an approved treatment. Often, even if there is an available treatment, patients may face financial difficulties or other access barriers to these life-saving therapies.

While these challenges can be frustrating, even for those skilled in navigating the medical system, Ramsey-Paige’s experience motivated her to help others impacted by rare disease.

“I knew I could use my personal experiences, along with my training, to hopefully make a difference in the lives of people with rare diseases,” she said. “And that’s exactly what I’ve done.”

Ramsey-Paige began her nursing career working at academic medical centers, eventually focusing on rare disease patients with unmet needs, including those with a rare, genetic disorder called hereditary angioedema (HAE).

Today, she uses her expertise as a Senior Global Nurse Advocate for BioCryst, a global biopharma company based in Durham, NC, that develops novel oral medications for rare diseases, including HAE, where she’s proud to work alongside others committed to addressing the needs of rare disease patients.

Important lessons

Rare diseases impact about 5% of the worldwide population, so chances are, you may know someone with a rare disorder, even if you don’t realize it. So here are a few things Ramsey-Paige wants everyone to know:

  • A rare disease is something someone has, but it is not who they are. Some rare diseases require constant care and are readily apparent, while others can be more easily managed behind the scenes, but it is only one aspect of a person’s life; it doesn’t define them.
  • Inequities only compound the challenges many patients face. In addition to lack of awareness about the diseases themselves, there is also often insufficient understanding of the limitations faced. Proper accommodations in the workplace, at home, in educational settings and more can go a long way in helping those with a rare disease.
  • Greater awareness and continued momentum are needed. I’ve seen progress in multiple disease states, however, there is still much to be done, including in the areas of policy and access, as well as advancing the science of rare disease.

Rare strength

Perhaps most importantly, Ramsey-Paige wants people to know about the rare disease community’s strength.

“The rare community is incredibly strong and motivates me every day, from those bravely navigating their own rare disease journey to the caregivers taking on an often underestimated and underappreciated role,” she said. “The efforts of each and every member of the community matter greatly, whether it’s raising awareness, advocating for change or researching new treatments. Even one determined person can move mountains.”

How one woman’s rare type of ovarian cancer was almost overlooked

(BPT) – Richmond, Virginia, resident Tiffany Stout never realized that in the prime of her life she would not only be battling cancer, but also a cancer that is rare with limited treatment options. She quickly learned how to become an advocate for her needs and an educator to others.

Tiffany has a history of ovarian cysts, so she didn’t find it alarming when she experienced spotting and sensitivity in her abdomen. These were fairly normal symptoms for her that she thought would pass with her period as they had previously. But her symptoms persisted beyond her cycle and started to interrupt her daily life. “I noticed when I would run I would have pain in my abdomen, almost like vibrations, which made it difficult for me to exercise. I also noticed that I was retaining fluid in my love handle area. I thought I was just getting older but it was fluid from the tumor.” Frequent urination was also becoming a problem, interrupting her workday, so she reached out to her gynecologist, who urged her to follow up with her general practitioner for an ultrasound, since insurance red tape prevented her gynecologist from ordering it. When the results came back, the ultrasound revealed possibly cancerous tumors.

After a magnetic resonance imaging (MRI) test, Stout underwent surgery to remove the tumor and further testing revealed she had stage III low-grade serous ovarian cancer (LGSOC), a serious disease that often goes undiagnosed in young women like Stout.

LGSOC is serious and different

LGSOC is a rare type of ovarian cancer that disproportionately affects younger women ages 45-55, although some have been diagnosed as early as 14 years old.1 An estimated 1,000-2,000 new cases of LGSOC are diagnosed every year in the U.S.2 LGSOC represents 6-8% of all ovarian cancers.1

Despite being classified as a separate disease in 2014, more recognition is needed around the differences between the more common high-grade serous ovarian cancer (HGSOC) and LGSOC so that it can be diagnosed and treated sooner.3

LGSOC is difficult to diagnose, as symptoms — such as back or pelvic pain, feeling full quickly, constipation or other changes in bowel habits — can often be overlooked as not being related to cancer.4,5,6 This is particularly the case when doctors examine younger women as ovarian cancer is generally thought to be uncommon at their age.1 For these reasons, LGSOC is likely to be diagnosed when it has already spread outside of the ovaries.7 Yet for many, LGSOC and its treatment will have a long-lasting impact on mental and physical health, including fertility and early menopause.8,9,10

Stout’s journey to treatment

Stout’s diagnosis in her 30s was especially troubling because there was little understanding and information about LGSOC as its own disease, subsequently leaving many tough decisions in her court to make. “My doctor said it was low-grade and my treatment options would be impacted, but when I went to do my own follow-up research to make an informed choice about my treatment, there was not anything low-grade specific I could find. It all spoke to the more common HGSOC.”

Unfortunately, once diagnosed with LGSOC, there are limited treatment options available and these are based on other cancer treatment strategies.11 While the standard of care for ovarian cancer is chemotherapy, LGSOC is chemotherapy-resistant, meaning chemotherapy stops working for nearly all women with LGSOC.1,11

“I still had to go through chemotherapy to qualify for any clinical trials — a process that was difficult to navigate alone,” said Stout. “Even then, it took multiple medical opinions to finally get a management plan for my condition. I needed to remain constantly on top of my doctors and learned how to become an advocate for myself.”

Even with treatment, LGSOC is highly recurrent.12 About 85% of people who achieve full remission following first-line treatment will have their cancer come back.12 But there is hope on the horizon with promising treatment options being investigated to see if they can work for people living with LGSOC.

Awareness and patient advocacy

The LGSOC community has been working hard to raise awareness of the disease, advance research and provide resources that help educate and connect patients — but there is still room to build upon existing efforts.

“It can be challenging to balance hunting down information while also living your life, so it’s important for people to have better access to key information about LGSOC so they can make informed decisions about their management plan in partnership with their doctor,” said Stout. “I also wish that there were more tools that helped connect patients to support groups as it can often feel very isolating living with a rare disease like LGSOC.”

“Since LGSOC is a relatively newly recognized form of cancer, it is unfortunately not unusual that many doctors have never diagnosed a LGSOC case before and could be seeing it for the first time in their patients,” says Dr. Jubilee Brown of the Levine Cancer Institute. “This makes it especially important that women have resources available specifically for LGSOC and are encouraged to talk about their unique experiences with their doctor so they can figure out the best path forward together.”

For example, at a community garden in Detroit, homeless women and mothers gained confidence and self-reliance by learning how to grow their own vegetables.

Here are five vital ways gardening can enhance your well-being, along with just a few examples of how they’ve changed many lives for the better.

1. Boost your physical health

Working in your garden is a great way to be more active. According to the CDC, gardening is the second favorite physical activity of older adults (behind walking), providing moderate physical activity which may help reduce stress, lower your blood pressure and maintain a healthier weight — all factors that can reduce your risk of heart disease, among other benefits. Being outdoors helps you obtain vitamin D from sunshine, boosting your mood and contributing to your body’s ability to absorb calcium, which is crucial for bone health, according to the National Institutes for Health. Physical therapy in a garden setting is a proven technique for faster recovery times. As an added bonus, you can grow vegetables and fruits for a more nutrient-rich diet.

2. Make social connections

At a vocational therapeutic garden in Chicago, one young man with autism relearned how to communicate verbally after years of being unable to use his voice. This helped him socially, as well as enabling him to get and keep a job. To create your own social connections, try joining (or volunteering at) a gardening club, working in a community garden or chatting with a neighbor while you’re working in your yard.

Japan made this part of their national health program due to research showing the many health benefits of being in nature. The therapeutic benefits of gardens are widely appreciated, which is why you see therapeutic gardens at hospitals, rehabilitation centers, hospices and other facilities. Research reveals that those who spend time outdoors may experience better recovery, less anxiety or depression and reduced reliance on pain medication. Children in a homeless center in Dallas were given the opportunity to work in gardens, leading to a marked reduction in their aggressive tendencies caused by the uncertainty in their lives.

5. Elevate your experience

For many, spending quiet time in nature also helps foster a feeling of connection with all living things. Gardens have long been a feature of spiritual retreats and sanctuaries.
