(BPT) – When migraine strikes, it can mean days of excruciating head pain combined with nausea, vomiting, and sensitivity to light and sound for those affected. The World Health Organization considers migraine to be one of the top 10 most disabling illnesses, affecting a significant number of people — nearly 40 million in the U.S. alone. In addition to pain and suffering, migraine can also cause anxiety and stress for more than 90 percent of people who cannot work or function normally during an attack. In fact, mental health conditions like depression and anxiety are the most common secondary health conditions affecting those with migraine. Clinical studies show that depression is almost twice as common and anxiety disorders are two to five times more prevalent in people with migraine than in those without migraine.1

New Survey Supports Link

The 2022 Migraine and Mental Health Connection Survey was conducted by the American Migraine Foundation with support from Biohaven Pharmaceuticals to better understand migraine, its impact on mental health and how people with migraine (PwM) and their healthcare professionals (HCPs) view the relationship between migraine and mental health. The survey, which included 1,100 PwM and 302 healthcare professionals (HCPs), including neurologists, headache specialists and primary care physicians, found that both PwM and HCPs agree that those who are better able to successfully manage their stress and mental health conditions are also better able to manage migraine. Likewise, almost all PwM and HCPs believe the unpredictable and disabling nature of migraine attacks often create worry and anxiety that can make managing migraine more difficult and further impact mental health.

Judy Ho, Ph.D., triple board-certified clinical and forensic neuropsychologist and former co-host of The Doctors, notes, “Many of my patients feel hopeless and powerless because of migraine. For some, talking about something stressful or anticipating a stressful event can trigger a migraine attack. I educate them about the important connection between the body and the mind and encourage them to do what’s needed to manage both their migraine symptoms and their mental health symptoms, as they often influence one another.”

Conversations Around Migraine and Mental Health

The survey found interesting differences in perspectives between PwM and physicians. The majority of PwM recognize the importance of talking with their physician about mental health but feel worried to bring it up and wish their doctors would initiate the conversation. Less than half of physicians surveyed were aware of this concern. They also appear to underestimate the prevalence of depression and anxiety disorders in PwM. Although more than half of the PwM surveyed had a mental health diagnosis, physicians estimated these conditions occurred in less than one-third of their patients with migraine.

An important step in bridging the gap and improving outcomes is initiating more in-depth conversations between patients and their doctors. The majority of both PwM and HCPs agree that mental health benefits from improved migraine control and that those who are better able to manage their stress and mental health conditions are also better able to manage migraine. Further, PwM want their doctors to factor in mental health as a priority when treating them. The majority of both groups agree that migraine management needs to be customized to each patient’s individual needs.

“Closing the gaps in communication between healthcare professionals and their patients can help improve migraine and mental health management,” said Larry Newman, M.D., professor of neurology at NYU Grossman School of Medicine and chair of AMF. “I hope that the findings of this survey encourage HCPs and people with migraine to feel empowered to speak out about their pain and to have deeper, more meaningful conversations about migraine and mental health.”

This school year, prioritize your child’s whole health – physical and mental

(BPT) – As back-to-school season gets underway, it’s important to ensure that your child is ready for the school year, both physically and emotionally, by scheduling a well-child visit.

Annual well-child visits are doctor appointments for preventive health services which are essential for ensuring a child’s growth and tracking developmental milestones. The well-child visit is also the time for routine immunizations to prevent diseases like measles, polio, hepatitis B, chickenpox, whooping cough and other serious diseases.

“Like vaccines, which prevent physical health conditions, speaking with your child’s primary care physician regularly about mental health concerns is also an essential part of overall preventive care,” said Rhonda L. Randall, D.O. and chief medical officer at UnitedHealthcare. “Your annual well-child visit is also an opportunity to have a conversation with your child’s physician. It’s best to have these conversations when problems or warning signs first appear, so your physician can take the appropriate steps to best treat them.”

If you’re not sure what questions to ask your child’s primary care physician during an annual well-child visit, consider the following:

Ask what vaccines are appropriate for your child’s age — and how to make up any that have been missed. You can refer to the list of child and adolescent vaccines recommended by the Centers for Disease Control (CDC) at CDC.gov/vaccines. Some common warning signs that your child’s mental well-being isn’t where it needs to be include persistent sadness, withdrawing from or avoiding social interactions, displaying outbursts of extreme irritability, drastic changes in mood, behavior or personality, changes in eating habits, difficulty sleeping, frequent headaches or stomachaches, difficulty concentrating, changes in academic performance or avoiding or missing school.

Ask for guidance on how best to support your child. Whether you have concerns about your child’s nutrition, exercise, sleeping patterns or behavioral changes, your child’s primary care physician is a great place to start. With so many young children experiencing mental and emotional health challenges, it’s important to create opportunities for them to share how they are really doing. Remember that these can be sensitive topics for your child to discuss. Empathy and patience go a long way to help children and adolescents feel listened to and comfortable.

Don’t forget to bring your sports physical forms. It’s great if your child participates in school sports. The wellness visit is an opportune time to make sure your physician is aware that your child is a student-athlete and address any concerns like nutrition, prior injuries and family history.

Ask for recommendations for other health care professionals, if needed. For example, if your child hasn’t seen the dentist in a while, if their vision screening indicated that they need to see an eye doctor, or their mental health screening has raised concerns, ask which health care professionals in your plan’s network they would recommend.

“If you haven’t already, now is the time to schedule an appointment with their pediatrician, to give your child a healthy start to the school year,” added Randall. “Regular well-child visits are essential in making sure your child is up to date on immunizations and that their developmental milestones are on track — including their mental well-being.”

Cutting-edge research

New treatments are investigated through clinical research studies (also called clinical trials) which are scientific studies that evaluate an investigational medication in a certain condition. All medications must be tested in clinical research studies before they can be approved and prescribed to patients.

Research is ongoing to evaluate a new, potential option called navitoclax. Navitoclax is being studied in clinical trials to evaluate its safety and efficacy in the treatment of myelofibrosis. It is an investigational, oral BCL-XL/BCL-2 inhibitor. The BCL-2 family of proteins are known regulators of the apoptosis (which means “cell death”) pathway. People with myelofibrosis often have a high expression of BCL-XL and BCL-2, and the BCL-2 family of proteins prevent apoptosis from occurring.[5]

Navitoclax has not been approved by the U.S. Food and Drug Administration (FDA) or other regulatory health authorities. Safety and efficacy are under evaluation.

If you are over 18 years old with a diagnosis of myelofibrosis, you may be eligible to participate in one of these clinical trials.

Participating in a clinical trial is a personal decision. As with any new treatment under investigation, it may affect you differently from your current medication. You may also need more doctor visits which would necessitate more time and travel.

And finally, you could play a role in helping to advance the field of myelofibrosis research. Your participation in the study is voluntary – you can stop participating at any time for any reason. The investigational drug will be provided at no cost to you, and you will not be responsible for study procedures that are not part of the routine treatment for your disease.
